- Determine your current number of locations. Locations include the main site and any branches of home health agencies, home hospice and palliative care, hospice inpatient or residential units, private duty home care providers, durable medical equipment, and home infusion providers
- Renew your subscription by paying online via the secure shopping cart (above) or submitting a completed order form with credit card information, unless other arrangements have been made
- Submit an updated list of your selected site users using the fillable PDF file or submit an excel file with the same requested information to activate@homecareandhospice.com
- Provide access to additional users
- Send us your surveyor’s contact information for online access during your survey
